This report discusses eight commercial or self-developed, Web-based systems being used in public health, safety net and other healthcare organizations looking for better ways to ensure that patients referred for specialty or follow-up care actually make the connection. These applications make more organized referral programs possible by:
- Matching patients with providers willing to except their type of reimbursement
- Ensuring appropriate clinical matches
- Providing a reliable way to arrange and track referrals
- Enabling communication between referring and receiving providers
- Keeping and enabling analysis of referral processing data
- In some cases, actually enabling the booking of the appointment from the referral site
