Serious challenges, including primary care and specialist physician shortages, rising costs, and the misalignment of reimbursement, resources and quality of care, are forcing healthcare providers, payers and industry regulators to transition to a next generation of ambulatory care. This next generation requires new care delivery models, reimbursement strategies, and care process improvements. It also entails transitioning care that does not require physician involvement to community agencies, consumers that are prepared to mange and provide more of their own care, and better use of technology that enables these changes.
